Student Mentorship & Goal Setting Forms
I am where I am today as a result of countless mentors who have helped light my path, and illuminate my future. As a result of these experiences, I have formalized some of these important mentorship “building blocks” I wished I had access to during my formative years and provided them FREE for my students and their families here.
Parents interested in learning more about how to use these forms, and how they can help you or your child progress towards their goals can contact me anytime.

Enrolled Student FAQs

Come to class anyways. You will be loaned a white belt for the day as a reminder of the importance of being responsible for your possessions.
BY going through this exercise, students are educated on the importance of not forgetting their belongings which becomes more important as they grow (keys, passports, drivers licenses etc.)

Notebooks and water bottles are welcome in class. Keep them at the back of the dojo please and thank you.
-
a mouth guard is not essential when you start your training, but it will be required soon after.
I do not promote a specific brand of mouth guard, so it is up to you, your dentist, orthodontist, etc!

Students are expected in every class with their gi pants and belt. From May-October, students may wear a plain white t-shirt or their “Summit” dojo t-shirt instead of their gi jacket. Students are expected to be in bare feet for all classes to prevent slips, slides and falls unless socks are medically indicated.
